Who May Qualify

Clients may qualify for care management services if they need assistance coordinating healthcare, home care, behavioral health services, community supports, benefits, housing, transportation, or other services necessary to remain safe and stable in the community. Eligibility may depend on Medicaid status, medical conditions, disability status, functional needs, and the specific New York State program.

A client may be eligible if they have one or more of the following:
